RESPONSIBILITIES

• Perform community profiling and identify the community needs and assets within the central/north region of Singapore
• Engage with relevant social and healthcare stakeholders to develop and pilot workflows, protocols and process to facilitate the social-health integration
• Setting up linkages and protocols between different community providers to manage clients across multiple social-health services
• Review and track the effectiveness of interagency workflow in facilitating care transition and provision
• Develop and implement new initiatives to meet the social and health needs of the community

Job Requirements:

RESPONSIBILITIES

The role will be part of the Family Support Services-Families for Life team playing a trainer, delivering evidence-based parenting and marriage programmes to parents and couples. It works collaboratively with schools and community partners in planning of these programmes as required, and will be involved in various community outreach efforts.

 Roles & Responsibilities:

1. Case Management:

• Manage 12 to 15 parenting consultation cases per month
• Provide counselling, programme, and case management services, including information & referral for identified clients
• Conduct client assessment and intervention plan based on programme requirements
• Participate in networking and case management meetings, if needed
• Ensure required case documentations are properly completed in a timely manner
• Application of organisational frameworks that include resilience capacity and risk assessment

Front Desk Administration and Secretariat Support: 

• Greet and attend to public walk-ins. This may include administering visitor logbook and badges, notifying relevant colleagues or re-directing public to appropriate resources
• Answer, screen and route incoming phone calls to relevant parties, taking down phone messages, checking of incoming phone messages and emails

Care Corner Learning and Special Needs Support is looking to expand our focus from just educational therapy to providing wholistic support for children with learning and special needs. We are seeking a pool of Specialists on a part-time/flexi-time basis with a passion to supplement our full-time in supporting children with learning and special needs. This pool  of specialists would ideally have experience teaching children individually or in small group sizes, open to working from different locations and be willing to support our programmes over a minimum period of 6 months.
