1. Conduct means testing for clients.
2. Process fund applications ensuring that all necessary documents are in order and submitted for approvals in a timely manner.
3. Network with various agencies and individuals to mobilize social resources to meet patient’s needs.
4. Identify presenting or underlying issue and utilizes available resources to develop a case plan to enhance, maintain and promote psychosocial functioning.
5. Identify more complex psychosocial or bereavement needs and refer to psychosocial services (PSS) team when required.
6. Conducts home visits when necessary.
7. Advocate for the protection of patient’s dignity, confidentiality, and rights
8. Participate in multi-disciplinary discussions and case conferences.
9. Work with pharmaceutical companies for possible Patient Access Programme (PAP) and/or other collaborations.

Key Requirements:

• Degree, preferably related to psychology or social work
• Knowledge and skills in assessments and working directly clients (ie conducting interviews/home visits)
• Positive attitude
• Good communication skills, strong interpersonal skills
• Comfortable working with people of all levels
• Willing and able to work on weekends when there are events
• English and a second language (ability to speak dialect will be beneficial)

• Min 15 years of facilities management experience
• PMP certification is a bonus
