About the Role
As a Patient Care Assistant (Daycare), you will play a vital role in supporting the daily needs of patients and ensuring their comfort and safety. Reporting to the Assistant Director of Nursing and Daycare Manager, you will assist in maintaining a clean and safe environment while delivering essential care under the supervision of Registered and Enrolled Nurses.

Key Responsibilities:

• Ensure patient safety under the supervision of a Registered Nurse.
• Attend to patients’ self-care needs, including personal hygiene and feeding assistance.
• Assist with patient mobility, including transfers and mobilization.
• Serve as a van escort, helping patients travel to and from the hospice.
• Measure and record patient vitals, such as weight, temperature, and blood pressure.
• Manage dietary needs, including meal orders, serving meals, and assisting with feeding when necessary.
• Maintain and organize the food pantry, ensuring food quality and stock levels.
• Uphold infection control standards through proper hygiene, waste disposal, and isolation practices.
• Provide administrative support, including data entry and document filing.
• Report patient observations and occurrences promptly to the Registered Nurse.
• Ensure a clean and safe environment by maintaining high standards of housekeeping.
• Stay updated with industry guidelines and develop relevant skills for effective care delivery.
• Perform other duties as assigned by the Assistant Director of Nursing, Registered Nurse, or Daycare Manager.

Experience and Skills Required:

• Minimum of GCE N Level or Certificate in Healthcare Assistance or equivalent.
• At least 2 years of experience as a Patient Care Assistant.
• Passion for serving patients with a strong sense of responsibility.

Work Schedule and Details:

Job Summary:

Role

The Operations Executive/Senior Executive is responsible for managing the day-to-day operations of the home personal care service provided to clients. This includes ensuring the efficient delivery of high-quality care, compliance with regulatory standards, and effective coordination of community caregivers, resources, and client needs. The role demands strong organisational skills to ensure smooth operations, community caregivers’ performance, and client satisfaction.

Key Responsibilities

Operational Management

- Oversee the smooth running of home personal care operations, ensuring that services meet both CWA standards and client expectations.

- Coordinate schedules for caregivers and other personnel to meet client needs, ensuring adequate staffing levels.

- Monitor and track daily service delivery, ensure timely visits and adherence to care plans.

- Maintain proper documentation of client care, community caregivers’ schedules, and service records.

Staff Management and Coordination

- Supervise and support community caregivers, ensuring they adhere to CWA policies and provide high-quality care.

- Coordinate training sessions, ensuring community caregivers are fully trained and prepared to meet client needs.

Recruitment & Onboarding

- Assist in the recruitment process, onboarding, and orientation of new community caregivers, including candidate screening.

Service Quality Management and Compliance

- Ensure that all home personal care services are delivered in compliance with relevant healthcare regulations / standards, and CWA policies.

- Review of SOP(s) regularly to ensure accuracy and relevance, making necessary updates as required.

Client Relationship Management

- Act as the primary point of contact for clients, families, and community caregivers, addressing concerns, feedback, and inquiries promptly.

- Proactively identify opportunities to improve service delivery and enhance client satisfaction.

Administration Support

- Assist with filing documents, maintaining accurate records, and organising client and family caregiver data in both digital and physical formats.

- Manage Case Management System (CMS) for home personal care service.

- Timesheet Management: Ensure the timely collation and submission of caregiver timesheets for payroll processing, resolving discrepancies where necessary

- Payment Collection: Assist with collection of cash payments from clients, requiring individual to liaise with clients on their payment or arrange with them on payment methods.

Other Duties

- Project Management: Participate in department initiatives and projects aimed at improving operational efficiency, caregiver performance, and client satisfaction

Requirements:

- Degree in Business Administration, Healthcare Management or related field.

- Minimum 1-2 years of experience in a community health or home care setting and understanding of home care services.

Halfway House hosts 100 offenders or abusers placed under community based rehabilitation.

Our skills development programs aim to provide the tools for a seamless re-integration in society.

Job Description

• Ensure daily needs of residents are well taken care of
• Motivate residents through their day to day interaction
• Oversee security screening to ensure contrabands are not brought into the house.
• Facilitate the process of residents checking in and out of the halfway house on a daily basis and provide daily updates on the headcount and other activities/incidents (if any) throughout the tour of duty. Any unusual movements should be highlighted and reported to the relevant parties.
• Refer residents’ issues to Social Worker when necessary
• Assist in institutional programmes such as outings (optional)
• Oversee Halfway House estate management
• Assist in monitoring of workplace safety and health standards.
• Ensure the overall safety and well-being of the residents during the programmes and activities that are carried out in-house and externally.
• To ensure that there is seamless transition during the handing and taking over of shift duties. The jobholder is also responsible in ensuring that all outstanding matters have been clearly communicated to the next duty personnel and are promptly followed up with.
• Work closely with Programme/Social Work team in the case management of residents, including monitoring and maintaining proper records on the progress, movement, and other relevant information pertaining to the residents.
