The Senior/ Physiotherapist develops and provides rehabitative care and services based on the care model, to residents of the Nursing Home(s). You will have opportunities to work with other healthcare professionals to design appropriate rehabilitation programmes for the residents of the nursing home. What you will perform:

• Assess participants through clinical history, physical evaluations, and outcome measures.
• Develop and implement client-centered SMART goals and treatment plans for participants and caregivers.
• Deliver physiotherapy interventions, including functional mobility training and client education.
• Design and oversee individual or group exercise programs, or guide Programme Assistants/volunteers in conducting these programs.
• Provide caregiver education and home therapy as needed.
• Support the introduction of new clinical services and therapies.
• Engage in continuing education, research, and quality improvement initiatives, including in-service training for staff and volunteers.
• Document participant progress and care outcomes.
• Perform other duties as assigned by the HOP or Lead Therapist.

What we expect:

• At least 2 years' in Physiotherapist with experience in senior dementia.
• Fully registered with Allied Health Professionals Council (AHPC).
• A self-reliant individual with minimal supervision.
• Looking to enhance own skillset, take in challenges and to play part of pioneer team in assisting future expansion.
• Bilingual in MANDARIN and ENGLISH to liaise with Mandarin-speaking clients.
• Singaporean only.

The Home has 14 separate Resident Living Areas (RLAs), each designed as an individual home-like environment. Hope RLA is specially designed to meet the needs of relatively independent residents with dementia.

RESPONSIBILITIES

About the Job

To provide for the physiotherapy requirements of the Nursing Home residents in accordance with the Allied Health Professions Council guidelines and The Salvation Army's Mission and Philosophy of service provision.

Key Responsibilities

• Conduct initial assessments for residents new admission
• Conduct 6 monthly Care Plan and programme review
• Prescribe and execute individual and group therapy programme and activities for residents
• Able to assist and engage residents with behaivour of concern
• Review and revise programme schedule for therapy program with proper documentation
• Liaising closely with other healthcare professionals to encourage a holistic approach to Care
• Maintain an inventory list of therapy equipment and ensure regular maintenance is done
• Participate in multidisciplinary case discussions
• Participate in continuing education, training and research activities when opportunity arises

• Bachelor degree / Diploma in Physiotherapy
• Full registration with APHC

Preferred Candidate Attributes

• Minimum 1 years of working experience in ILTC sector

Working Hours

Under normal circumstances the hours of duty will be as follows:

8.30am to 5.45pm (Monday to Friday, 45 minutes Break inclusive)

We are looking for Physiotherapist to be a part of our dynamic trans-disciplinary team of Allied Health Professionals which comprises Occupational Therapist, Speech Therapist, Physiotherapist, Music Therapist and (Associate)/Psychologist. The team provides a range of rehabilitative and intervention programs to support our client's ability to participate in the home and community.

Job Responsibilities:

• Assess and identify the individual needs of clients andformulate, develop and implement appropriate individualised therapy programmes
• Monitor training programmes and review its effectiveness
• Liaise with various disciplines involved in the rehabilitative treatment process
• Refer clients to the respective specialists forconsultation and intervention

Job Requirements:

• Recognised Degree in Physiotherapy
• Eligible to register with the Allied Health Professions Council in Singapore as Conditional / Full Registrant.
