• Assist in evaluating care ecosystem efforts (e.g.: programmes, workflows)
• Develop publicity materials and plans with the support from Corporate Communication representatives

Partnerships Engagement

• Support the implementation of new initiatives and coordinate partner engagement efforts
• Create an asset map of community partners to identify service gaps and strengths
• Collaborate with partners and government agencies, and attend networking events as needed

Administrative Work

• Collect and analyse data, document learnings, and ensure timely report submissions
• Ensure accurate record-keeping and report submissions in a prompt and timely fashion
• Support evaluation and reporting processes for management and funders

Qualifications

• Min Diploma/Degree in Social Work, Community Development, Counselling or Psychology preferred
• At least 2 years experience in programme planning, care coordination, or community work
• Prior exp in community partnership, familar with Active Ageing Centre
