• Interpret the Contract and extract all essential information with the project team for the efficient and effective management of the project to meet its objectives

What's the Role?

While managing a team of Workflow Specialists, you will lead by example by conducting detailed client conversations and reviews while ensuring the team is operating at your standards or above. As a Case Manager, the job requires you to:

1. Create and Individualised Care Plan for each and every resident; and work towards addressing their psychosocial needs through care and counselling, eventual rehabilitation and reintegration into the community.
