Requirement

• Min. Diploma/Degree in related industry such as healthcare, social or community services sectors.
• Min. 3 years of working experience in healthcare, social or community services sectors.
• Prior healthcare or social services qualification such as nursing or allied health/social services-related experiences would be good
• Case management experience is an advantage!
• Keen to work in the community setting.
• Has the passion for care of the elderly, including counselling and working with the family on social issues such as discharge care planning.
• Must be matured and sensitive in working with the patients, family members, the public and the healthcare partners.
• Able to work in collaboration with the healthcare partners in the hospital’s multi-disciplinary team and outside the hospital.
• Able to work independently as well as embrace teamwork.
