Requirements:

• Minimum 8 years of engineering experience with a recognized degree in either Civil/Mechanical/Railway Engineering of which, at least 5 years of experience in managing construction team for railway projects of similar scale
• Proven technical, engineering and managerial experience and be conversant with all relevant rules, regulations, codes of practice and Company Standards

Job Responsibilities

Case Management

• Assess the community health and social care needs
• Coordinate support and service placement (where required)
• Develop and pilot work flows, protocols and process that facilitate the integration of social and health care for residents
• Develop and implement calendar plan of social and health activities for seniors, with seniors to promote a community climate of multi-racial tolerance, harmony and co-operation among the residents
• Employ strengths-based practice model to work with the service users:
• Identify service user’s physical, emotional and social needs
• Conduct care assessment for service users
• Formulate Individualised Care Plan (ICP) for each service user with relevant stakeholders
• Help service user reach goals and action plans through focusing on personal and environmental strengths
• Follow up to monitor their function, psychological, social / environment status review the ICP with the service user on at least 6 monthly intervals identify, stocktake and assess the strengths and limitations of community assets, including other Community Care service providers
