• Manage ESRD patients in dialysis centre or home dialysis (HD/ PD) through client engagement, conducting interviews, data collection, case assessment (BPSS, risk & needs assessment and other relevant assessment tools), case conceptualisation, case planning and intervention plans for best patient outcomes
• Identify presenting/underlying issues and problems to design a care plan to enhance/maintain and promote psychosocial functioning of patients
• Conduct FFRs (Financial Fee Reviews) and placement on NKF subsidies and welfare programmes
• Conduct visits to dialysis centre, home or hospitals to provide supportive counselling
• Advocate for the protection of patient's dignity, confidentiality and rights
• Collaborate with healthcare professionals professionally internally and externally in total patient management
• Network with government, community, social agencies or individuals to mobilise social resources to meet patient's needs
• Facilitate patient and caregiver support group programmes
• Document bio-data of patients and interventions in clinical system for records, complete timely online case documentations (Case session recordings, follow-up recordings, risk and needs assessment, social reports, patient feedback forms etc)

Qualifications

• BA in majoring Social Work/Graduate Diploma in Social Work
• Registered Social Worker (RSW) with the Singapore Association of Social Workers

Experience

• Experience in social service agency, with at least 3-4 years in direct social work practice
• Candidates without experience are welcome to apply

The Children and Youth Group (CYG) in The Salvation Army Singapore serves hundreds of vulnerable children and youth yearly. In line with our vision to provide a continuum of care to uplift those in need, our services span from preventive services, foster care and residential services to reunification support services for vulnerable children returning to their natural families after being removed due to safety concerns. The Youth Development Centre is one of CYG's services and reaches out to latch-key young people from low to middle income families to guide them towards a more purposeful life, live out their dreams and contribute well to society.

As an outreach centre, the staff and volunteers engage the youth with a variety of sports, music and social activities to impart life skills that will help them build a strong foundation for their future. We run a drop-in centre to provide young people in the community with a safe and conducive place to hang out. We also run youth mental health programmes such as resilience building to help youth thrive. Through our youth mental health programmes we reach out to youth in the community as well as at risk youth in residential care. We also provide supervised access services for vulnerable children and youth.

Further, Youth Development Centre is also an incubator for new services such as youth mental health.

RESPONSIBILITIES

About the job

The Youth Worker (YW) will be responsible for working closely with key stakeholders to plan, organise and implement these programmes which will involve managing volunteers, engaging youth from the community.

You will need to conduct needs and outcome assessments to measure and enhance programme effectiveness and ensure that our programmes are run well.

Other than these programmes, the YW will anchor the running of the drop in centre, including engaging and mentoring children and youth, organising or conducting activities, working with volunteers and partners and conducting youth outreach.

We are looking for someone who wants to make a difference in the youth mental health space, with a "Can Do" spirit, is a creative problem solver and has a passion for making a difference to youth. You should have the special X factor to connect and inspire youth.


Job Responsibilities:

• Work with young people in accordance to YDC approved protocols while maintaining professional and purposeful relationships in order to enable the youth's holistic development.
• Implement and run youth mental health programmes including resilience building groupwork.
• Ensure smooth running of daily operations of YDC, from planning to execution of programmes that will develop youths' mental health coping, social and emotional competencies.
• Be the main liaison person to reach out to, communicate and work with potential stakeholders such as organisations, corporates, vendors and volunteers, to bring in appropriate programmes that meet the needs of our youths.
• To ensure volunteer-initiated programmes align with the different needs of the youths while maintaining the interest of volunteers.
• To engage and harness the support of corporate sponsors and community stakeholders towards the fundraising efforts or initiatives of the Centre and CYG.
• Strategise with the team on how we can effectively reach out to youths who would benefit from the programme via traditional means, partnerships and social media.
• To plan, develop and execute programmes that caters to the physical, emotional, spiritual, educational and social needs and interests of our youths. This includes youth mental health programmes and services.

Diploma or Degree in Psychology, social work or any other relevant discipline from an accredited institution.

Job Requirements

• Experience with working with youth mental health preferred.
• At least 1 year of experience working with youth.
• Flexible and open to new initiatives as and when YDC incubates a new service.
