Our goal is to provide suitable intervention care for seniors. This is done with a team of trained social workers using a case management approach, monitoring and counselling services. As a Social Worker, you will be required to carry out casework and apply intervention strategies appropriately to improve the well-being of the client and their families/caregivers.
You will need to
1. Complete Basic Casework and Counselling:
- In consultation with the program lead, conduct assessment, planning, and intervention for cases
- Engage clients and build rapport
- Develop and review appropriate intervention strategies
- Perform needs assessment
- Advocate for clients' needs
- Coordinate services with other community resources on the cilents' behalf
- Prepare case notes
- Manage case referrals
- Conduct and participate in case conferences/MDT meetings
- Monitor delivery of coordinated services
- Document and evaluate case outcomes
- Adopt a case management approach when working with clients
- i.e. collaborate with internal organization department, and other agencies/specialists to provide multi-disciplinary assessments and monitoring of clients
2. Share Information and Conduct Referrals
- Refer and coordinate care services for clients to ensure they are adequately supported to continue living in the community
- Provide information on the various assistance schemes or care options available
3. Support Administrative Functions
- Maintain and update progress reports and BumbleBee (software)
4. Additional Responsibilities
- Raise awareness and facilitate Advance care plan for seniors
- Conduct outreach activities to the community
- Other ad-hoc assignments
You should
- Have good knowledge of the geriatrics and community services
- Be proficient in MS Office applications
- Good communication skills (oral and written)
- Have strong assessment and problem-solving skills
- Be detail-oriented
- Be comfortable working independently
It would be great if you
- Are able to converse in local languages
QUALIFICATIONS
- Graduate Diploma or Bachelor's Degree in Social Work
- Minimum 3 years experience in community healthcare settings in Singapore
- Possess 1-2 years of experience in administration
