Executive (Partnership Development & Project Management) , Senior Care Divsion

Job Description:

Partnership

• Assist Manager to draw up partnership agreements (eg. MOUs) for review and approval and successful implementation of partnership workflow/processes and systems. (eg set up of agreed workflows between partners for the partnership)
• Coordinate with other team members within Sparkle Care and external partners to ensure all aspects of the core partnerships can work effectively e.g., facilitate seamless delivery of the continuum of care between the inhouse and external AAC, SCC and the HPC+ teams.
• Collect and compile KPI information for data reporting and KPI Submission on a weekly/monthly basis.

Active Aging Centres

• Cover day to day operations in the Eldercare Service at the designated Senior Care Centres or newly awarded Active Ageing Centres as required.
• Deliver a high level of customer service and engagement with seniors and community partners.
• Maintain proper records and documentation of networking and collaboration, incidents and phone calls/communications.

Volunteer Management

• Contribute to Sparkle Care’s Volunteer Management effort, where needed.

Other Duties

• Assist in the review of programmes and service offerings from time to time to keep up with the development in the eldercare sector.
• Participate actively in ad hoc projects to improve service quality and brand name of PCF Sparkle Care
• Any other responsibilities as assigned by the Reporting Officer

• Bachelor’s degree or diploma from an accredited University or Polytechnic
• Preferably with at least 1 year of working experience in an eldercare setting or befriending and buddying services [or volunteering services]

· Process and manage referrals for ACP discussions from inpatient, outpatient and community settings of SLH

· Facilitate ACP discussions with patients, families and nominated spokespersons

Department Overview:

Epworth HomeSweetHome was started in 2013 in partnership with the Ministry of Social and Family Development (MSF) as the first Therapeutic Group Home in Singapore. The Home provides stay-in treatment for children between the ages of 7 to 12 years old who have experienced abuse. We use the Trauma System Therapy (TST) treatment model to help children with complex trauma and aggressive behaviour.

We believe that with early and appropriate intervention, children can recover and become well-adjusted persons with a healthy mental well-being. Apart from treatment within the Home, we also work with other helping professionals so that all the needs of these children could be met. Together with MSF, we work towards safe placements for these children, when they are ready to be discharged from the Home and well enough to return to the community.

Coordinate and responsible for the planning, execution and evaluation of programmes and ensure smooth running of daily operations of the department, which includes:

DUTIES AND RESPONSIBILITIES

1. Programme Planning and Implementation

• Develop and implement outreach plan to seniors.
• Develop and implement annual calendar plan of social and health activities for seniors.
• Ensure that the programmes and activities meet the performance indicators and operating guidelines issued by Agency for Integrated Care, administration of the CSQ, half-yearly progress report on the programmes and services, outcome management, etc.
• Conduct needs assessment survey if necessary.
• Initiate a case record file for every elderly household in the identified block and capture the needs of the seniors.
• Establish and motivate seniors to be active, healthy, interdependent and purposeful.
• Ensure that the activities and programmes of the centre promote a community climate of multi-racial tolerance, harmony and cooperation among residents.
• Network with other agencies to establish additional avenues of help for the welfare of the seniors.

2. Alert Alarm System

• Oversee and manage the Alert Alarm System (AAS) during the centre’s operating hours, rendering assistance when necessary.
• Establish a system to manage the AAS outside operating hours and ensures that help continues to be available when needed, including a system of recalling staff outside operating hours.

3. Volunteer Development

• Establish networks with local community-based services/ resources, such as grassroots, social services, corporate organisations and schools, to engage volunteers to help run or assist in the programmes, start income-generating activities, conduct outreach efforts etc., to match the needs of seniors.
• Recruit, interview and screen potential volunteers.
• Provide and coordinate orientation of volunteers to their assigned role and the mission and objectives of the centre.
• Place volunteers to suitable positions and areas of need, such as administration of CSQ etc.
• Supervise and support volunteers.
• Establish a system of recognition for the volunteers.
• Review and evaluate the volunteer programme to ensure relevance and to meet future needs.
• Serve as the key communication link between the centre and its volunteers.

4. Finance and Accounts

• Manage the monthly petty cash utilization of the centre.
• Implement checks and controls in the accounting system to ensure proper accounting records and also to safeguard against misappropriation of the centre’s assets.

This role is responsible for the care management of elderly clients under the maintenance day care
programme, including assessment, planning, implementation and evaluation of nursing care as well
as client education. The main responsibilities include:
• Take charge of the whole clinical nursing practice in the centre and ensure compliance with
approved standards of nursing practices, including the creation and maintenance of policies
and SOPs.
• Perform all general nursing duties to provide quality nursing care and support to clients to

Key Responsibilities

• Provide daily escort duties for clients from their home to the hospital or polyclinic for their medical appointments.
• Accompany the clients on the vehicle and keep an eye on them during the journey.
• Assist client with walking aids or on wheelchairs with their ambulation.
• Assist the clients throughout their hospital or polyclinic visit, while ensuring their safety at all times.
