You will be responsible for planning and managing care integration initiatives driven by the SingHealth Regional Health System (RHS). You will provide and coordinate operational and administrative support to SingHealth RHS and its cluster stakeholders to ensure smooth and effective operations of the health promotion and disease prevention initiatives.
You will assist to provide analytics and data on RHS efforts to identify, plan and develop new population health projects and capitation initiatives. You will collate and track performance indicators of the care integration programmes and initiatives driven by the RHS.
You will liaise with RHS internal stakeholders, including clinical and operational departments from SingHealth institutions that are involved in population health projects and capitation initiatives driven by SingHealth RHS. You will cultivate and maintain strong networks with key community healthcare services and partners including Voluntary Welfare Organisations (VWOs), allied health and home nursing services, as well as other relevant social services and community-based organisations. You will also work with business units to understand data requirements and establish criteria to collect the relevant information for analysis and support data collection and reporting of key performance indicators for the RHS programmes, population health projects and capitation initiatives.
In addition, you will serve as a project facilitator for approved projects and support their development and implementation. You will apply appropriate problem solving tools and skills to help resolves issues and cross hurdles . You will provide administrative support, assist in the annual work plan exercise, plan and organise RHS retreats, events and provide secretarial support.
Job Requirements
· Bachelor’s Degree in any discipline. Experience in healthcare would be an added advantage.
· Strong analytical skills with an understanding of financials, statistics and workflow engineering and re-engineering would be an added advantage
· Good presentation skills and the ability to write well
· Effective and proactive when working both independently and in a team setting
· Excellent interpersonal communication skills, willing to learn and is self-motivated
