Key Responsibilities:

• Social & Financial Assessment: Perform social and financial assessments of patients and provide appropriate recommendations.
• Counselling & Family Conferences: Offer counselling to patients and their families, and take a leading role in family conferences.
• Collaboration & Networking: Work closely with relevant organizations to ensure comprehensive care for the elderly, fostering collaboration in patient care.
• Support for Destitute Patients: Address the social needs of patients who are destitute, ensuring they receive the necessary care and support.
• Multi-Disciplinary Team Participation: Contribute to the patient-care team, ensuring social work perspectives are integrated into treatment planning and care delivery.
• Groupwork: Conduct groupwork with elderly patients or their caregivers, offering support and resources to improve their well-being.
