Job Summary:
Ā· Right site patients and achieve optimal utilization of SLH services
Ā· Ensure expedient processing and managing all applications for admissions in a timely manner
Ā· Implement policies and procedures that improve efficiency of the Referral Centre
Ā· Support joint initiatives and collaborative projects undertaken with other healthcare and community partners
Responsibilities and Duties:
Ā· Conduct screening of referrals and assess suitability for transfers to SLH
Ā· Participate in clinical and/or quality improvement initiatives
Ā· Review and implement clinical protocols, processes, procedures and policies
Ā· Collaborate with other hospitals and strategic partners to develop and refine workflows to ensure seamless and expedient transfers
Ā· Participate in new projects and initiatives, where there may be impact on clinical services of the hospital (inclusive of providing administrative support in clinical research)
Ā· Assist in the preparation of reports and statistics as required
Ā· Assist in identifying problem areas, recommend and implement improvements
Ā· Perform other duties as assigned
Job Requirements:
Ā· Minimum qualification of a clinical degree or equivalent, preferably with at least 5 years of work experience in geriatric setting.
Ā· Able to work independently
Ā· Good collaborative and communication skills
Ā· Demonstrated ability and interest in working with the elderly population
