Responsibilities and Duties:

• Conduct screening of referrals and assess suitability for transfer

• Participate in clinical and/or quality improvement initiatives

• Review and implement clinical protocols, processes, procedures and policies

• Collaborate with other hospitals and strategic partners to develop and refine workflows to ensure seamless and expedient transfers

• Participate in new projects and initiatives, where there may be impact on clinical services of the hospital (inclusive of providing administrative support in clinical research)

• Assist in the preparation of reports and statistics as required

• Assist in identifying problem areas, recommend and implement improvements

• Perform other duties as assigned

Job Requirements:

• Minimum qualification of a clinical degree or equivalent, preferably with at least 5 years of work experience in geriatric setting.

• Able to work independently

• Good collaborative and communication skills

• Demonstrated ability and interest in working with the elderly population
