Sun Ray Scheme

Sun Ray is a centralised scheme administered by NCSS to develop individuals who have strong leadership qualities to lead professional practices and agencies in the social service sector.

To enable a holistic development. there will be structured rotation across social service agencies in different sub-sectors to provide individuals with cross-organisational exposure and support. The exposure will enable those on the Sun Ray to develop broader sector perspectives and gain deep sector knowledge to help the social service agencies that they are deployed to build capabilities and capacity.

We are looking for individuals with strong motivation and leadership capabilities to lead agencies or professional practices to achieve social impact in the social service sector.

Job Description

• Perform assessment of clients' physical mobility and falls risk
• Develop and implement care plans for clients and their care-givers with other clinical staff
• Provide appropriate physiotherapy interventions and based on assessment of clients
• Conduct functional mobility training including the prescription and use of appropriate mobility/walking aids
• Plan, implement and evaluate individual and group physical activity and exercise programs
• Conduct and/or supervise group therapy and exercises for the clients
• Conduct education and training for care-givers
• Maintain close communication on the clients' progress in the centre
• Conduct in-service and on-the-job training sessions for staff and volunteers

Requirements

• Registered with Allied Health Professions Council
• Relevant Diploma/Degree in Physiotherapy
• At least 2 years working experience
• Interested in rehabilitative and geriatric care and enjoys working with the elderly
• Maintain documentation of clients' progress and outcomes of care provision
• Supervise Therapy Aides/Assistants in the provision of therapy services to clients
• Cheerful disposition with good communication skills

• Assist in the implementation and promotion of new clinical services in therapy.
• Communicate the assessment results, therapy plans, goals and outcome measures to clients and other relevant stakeholders.
• Communicate the intervention plans, outcome measures, progress and discharge criteria to clients and relevant stakeholders.
• Conduct physical, psycho-social, discipline specific and impairment-based assessments to determine required therapeutic interventions.
• Conduct service and clinical audits.
• Deliver training.
• Document results of case history taking, assessment results, therapy plans, interventions, goals and outcome measures.
• Empower clients and caregivers on methods to enhance health, mobility and function.
• Evaluate history and physical examination findings using clinical reasoning to develop an appropriate client-centric intervention plans.
• Evaluate teaching outcomes at an individual level.
• Evaluate therapy progress and outcomes using selected therapy outcome measures at regular intervals.
• Facilitate community integration through collaboration with community partners as required.

As a Physiotherapist, you will be responsible for helping clients restore mobility, function and independence through detailed assessments of problems, individualised treatments and manual therapy. You will work in a multidisciplinary team to ensure a high standard of clinical practice and professional conduct in all areas of client care. You will also be in charge of organising and distributing the caseloads, supervising students, and coaching junior or support care staff.

Responsibilities:

• Provide quality and professional physiotherapy services for assigned caseload.

• Ensure monthly Rehab report and dashboard indexes are accurate and submitted and uploaded on time.
• Ensure a safe, clean, effective and efficient environment for all.
• Assist department to develop new initiatives and programs as directed.
• Ensure staff under direct supervision have attended the required training and are certified appropriately.
• Organise and participate in community outreach programs and when needed, give talks and lectures or mobilise team in educational activities.
• Be involved with in-service education for internal and external audiences.
• Involve in QIP and research projects: mobilise team to identify problem areas, recommend and implement improvement measures.
• Assist Centre in developing competency of all levels of staff.

• Full registration with AHPC with certificate of practice.
