• To perform home visits to chronic ill/ palliative patients

Job Description

• Responsible for ensuring person-centric care according to the needs of the residents and their respective conditions
• Perform nursing procedures in accordance to the person-centred philosophy of care, and in compliance with the home
• Work in close collaboration with other in-house functions to deliver holistic care
• Build rapport with residents and their families
• Provide supervision and clinical guidance to junior nursing staff
• Involved in orientation and on-going training of nursing staff

Key Requirements

• Diploma/ Degree in Nursing from an institution approved by the SNB (Singapore Nursing Board)
• Valid Practicing Certificate issued by the SNB
• Must have min 2 years of full-time/perm nursing experience in Geriatric ward or homecare settings (Must be recent!!)
• Advanced dip in gerontology would be a plus!
• Able to start immediately!
