Overview

The Position Holder will be responsible for engagement of partners and internal stakeholders to augment NUHS’ role as the population health manager for the western region. Job Responsibilities

You will be responsible for the following:

• Identify opportunities for strategic collaborations with key partners to improve care integration and augment community care
