Reporting to the Resident Physician, this role functions primarily as the case manager for SingHealth Community Hospitals (SCH) inpatients referred into SCH’s post-discharge programmes (e.g. Integrated Primary Care for At-Risk Elders [iPCARE]). • Chaperone and assist doctors in carrying out treatment and nursing procedures for patients.
• Dispense medications according to doctors’ prescriptions, supported with supervision.
• Cross-support nurses in their duties across each specialty discipline as required
• Any other duties as assigned by the Centre Manager

REQUIREMENTS

• Registered Nurse with Singapore Nursing Boar
• Min Diploma in Nursing or equivalent
• At least 2 years’ experience in a clinical setting, experience in Operating Theatres / AU will be an added advantage
