• Comply with ENHS requirements in care and documentation.
• Lead in response to emergencies, perform first aid and arrange for an ambulance when an emergency arises.

• Monitor the health and nutritional status of residents daily and escalate to Senior Nurse where applicable.

• Administer prescribed medications to our residents by adhering to the 10 rights.
• Observe for adverse side effects and the efficacy of the medicines.
• Documenting of IR in adverse reactions and medication errors.
• Evaluate the effectiveness of interventions and report to SN accordingly. (e.g. oxygen therapy)
• Record accurate and concise statements on the nursing care rendered to the residents in compliance with required standards.
• Monitor the nutritional status of residents and intervene where necessary.
• Encourage independence and provide support for residents’ hygiene care (e.g. skin care, oral care, bathing, dressing, grooming, toileting) when required.
• Provide necessary preventive (e.g. moisturiser) and therapeutic (e.g. positioning) skincare routines to residents to preserve skin integrity and prevent pressure injury.
• Perform ‘last office’ service to deceased residents and help with the transfer to Holding Room where necessary.
• Integrate nursing care and residents’ mobility, ADL community integration training where applicable. (e.g. walk to the toilet to help them change their diaper)
• Assist in the orientation to help residents and their families feel assured and welcome.
• Assess for signs of possible abuse and participate in the investigation as needed.
• Assess for signs of dementia or mental health issues, and report to SN where appropriate
• Participate actively in transdisciplinary discussions to promote quality care for residents.
• Coordinate the physical admission, transfer and discharge of residents to and from SLR.
• Schedule, arrange, and document any follow-up medical appointments residents might have.
• Partner with volunteers to provide the SLR model of care.
• Conduct OJT or in-service for care partners and new colleagues as assigned.
• Comply with environmental infection control policy.
• Record the use of chargeable clinical consumables.
• Support and work cooperatively with all team members based on the self-managed household concept for the residents which includes simple cooking, menu planning, light laundry, and housekeeping.
• Any other projects that are assigned by the manager.

Job Requirements:

• Minimum NITEC in Nursing and above.
• Newly attained upgrades from Nursing Aides.
• Registered with Singapore Nursing Board (SNB)

Job Responsibilities:

• Responsible for assessment, planning, implementation and evaluation of nursing care and service delivery.
• Conduct daily nursing rounds to assigned RLA to ensure maximum care and ascertain the need for additional or modified service.

Job Scope:

• Conceptualise and deliver virtual, blended and classroom learning interventions (e.g. develop learning resources/materials and monitor/evaluate impact of learning interventions etc.)
• Work with subject matter experts and trainers to shape the course content, curriculum and learning outcomes. Provide support and assistance to associate trainers and resource persons for the smooth delivery of programmes.
• Partner public agencies to develop and customise L&D interventions to support their developmental plans

RESPONSIBILITIES

1. Support team in monitoring of seniors' physical and mental health and well-being: 

• Conduct visual check and take/monitor vital signs 
• Act on clients' irregular conditions, report to senior care staff or supervisor and caregiver, if necessary, record in communication notes, submit incident report where needed
• Identify changes in seniors' well-being/behaviours, report to caregivers, plan and monitor Individualised Care Plan (ICP)  with Physiotherapist or Occupational Therapist.
• Respond to emergency and accidents. Use of PPE if required
• Administer medications following SOP, after being trained 

2. Provide high level of customer service and engagement:

• Plan and lead in exercises and activities for the seniors
• Serve meals and teabreaks
• Provide personal care to seniors to carry out their routine activities, being attentive to their needs and ensure their safety in the centre.

3. Ensure safety and cleanliness:

• Assist seniors in toileting.
• Mobility and transfer safety after being trained
• Follow infection control procedures, maintain cleanliness and safety of the premise
• Provide support as a transport escort to send and fetch clients to and from centre, if BCLS-trained. 

Minimum 1 year of working experience in healthcare/eldercare setting is preferred or possess any of the following:

• Diploma in Healthcare Management or Gerontology
• WSQ Diploma in Social Service
• SGUS programme for Care and Coordination (Community Eldercare)
• WSQ Certificate in Healthcare (Therapist Support or Nursing)

Requirements:

• Only Singaporeans
• Passionate and patience in serving the seniors
• Required to attend to seniors who are mostly mandarin/dialect-speaking. Proficiency in mandarin and/or dialects is essential
