Position title : Social Worker
Working venue: Active Aging Centres or Active Aging Care Hubs
Working Location: Location to be fixed , discuss more during interview
Working day: Mon - Fri | 8:30am to 6pm (5.5 days stated in contract (In case there is a special event)
Salary : $3000 - $4100
Job overview:
- The Social Worker (AAC) works in the field of Social Work, should qualified and trained to provide interventions and programmes, performing community development work, uplifting the professional practice in Social Work with the elderly.
- Conducts casework and group work as well as implement and evaluate community development activities and programmes to address service gaps in the community.
- Provides professional input to his colleagues as appropriate.
- The Social Worker works within the setting of the agency and its sister AACs, the community, Voluntary Welfare Organisations and healthcare agencies, and hospitals.
- Collaboration with other government agencies and ministries as necessary
JOB RESPONSIBILITIES
1. Case work
- Role in use of Community Screening (as part of AAC team)
- Role in Case Assessment
- Role in Intervention and Care Planning
- Care in providing general and clinical SW support to clients
- Role in Information and Referral
- Create and maintain casework records for elderly household in assigned areas.
2. Group Work
- Role in determining psycho-education / support / therapeutic needs seniors groups.
- Role in formulating SW group work in AAC
- Role in overseeing general group work / activity of AAC
- Role in the conduct of SW group work and general group activities
- Role in evaluating general group work in AAC
3. Community Work
- Assist in the determining of the community needs of the senior population within ACC boundary.
- Assist in the development of AAC Community outreach and Community development plans – for the short, medium and long term well-being of the seniors in the AAC boundary.
- Identify and establish network of resources within the community.
- Assist in the carrying out of AAC Community Work plan.
4. Programme Planning and Implementation
- Assist in the development and implementation of outreach plan to seniors.
- Assist in the development and implementation the AAC’s annual calendar plan of social and health activities for seniors.
- Ensure activities and programmes of AAC promotes local community aspirations.
5. Others
- Alert Alarm System: Assist in the monitoring and management of the Alert Alarm System (AAS) during the centre’s operating hours, rendering assistance when necessary.
- Volunteer Development: To assist in the development and coordination of volunteer activities.
- Training and Development of Personnel: Lead, motivate and train staff volunteers where possible.
- Self Care Plan: To develop and implement an approved Self Care, in order to provide optimum service standards for the client, colleagues and agency .
Job Specifications:
- Degree /Post Graduate Diploma in Social Work
- RSW is preferable or in process.
