RESPONSIBILITIES
Job Description
- Receive and process applications from AIC & CGH or other outside sources of referral
- Provide administrative support and coordination on new admissions
- Coordinate and setting up weekly/monthly Multidisciplinary Team meetings (MDM) and to follow-up when required
- Coordinate Family Care Conference and Client Care Conference
- Conduct and follow up in means testing
- Prepare documents for Medifund and any other financial assistance applications if necessary
- Coordinate home assessments and conduct home visits when necessary
- Post discharge follow up
- Provide administrative support
QUALIFICATIONS
- O/A Levels, preferably with certificate of office skills
Job Requirements
- Preferably with 2 years relevant working experience
- Have good communication and interpersonal skills and excellent telephone etiquette
- Should be a team player who is patient, warm and helpful and organised at work
- Knowledge of software packages such as Microsoft Office and Excel is required

Working Hours
Under normal circumstances the hours of duty will be as follows:
- 8.30am to 5.45pm (Monday to Friday, 45 minutes lunch break)
