The Metta Home for the Disabled and Metta Home Day Activity Centre (慈光乐陶苑及智障日间活动中心) were set up in December 2000 to offer community care options to adults with intellectual disabilities who are aged between 18 and 55.
Funded by the Ministry of Community Development, Youth and Sports (MCYS), the former is a purpose-built facility designed to accommodate 103 residents, whereas the latter is supported under the MSF and Tote Board Social Service Fund to provide day care services for a capacity of 50 beneficiaries.
Our programmes aim to equip all recipients with the skills and means to lead independent and fulfilling lives, so as to achieve social integration and quality of life.
Tending them with holistic care and working closely with their primary caregivers to provide well-rounded support is a multi-disciplinary team comprising a social worker, training officers, staff nurses, assistant nurses, nursing and therapy aides.
Job Responsibilities:
1. Social Work Tasks
- Review and update means testing results and subsidy levels of clients
- Assists in monitoring of outcome management and client satisfaction data
- Home/ hospital visits of clients in support of Registered Social Worker
- Understand and assist in the application of relevant assistance schemes/ community resources for clients & caregivers
- Assist in caregiver /staff training for psycho-educational programs or workshops
- Assists Registered Social Worker for admissions and discharges as well as case conferences
- Assists in complying to MSF/SG Enable guideline/requirements
- MFEC cards application for ADH
2. Administration Tasks
- Assist in audit/ licensing requirements of both DACs and ADH
- Maintain client database, case files and funding/subsidy records
- Submission of VWOTS
- Submission of Muster monthly financial to HQ for ADH and both DACs.
3. Others
- Participate in community/ volunteer events to network with partners
- Any other tasks as assigned by Registered Social Worker/Manager
- Assist in clients event/outing
Job Requirements:
- Minimum 1 years of related experience
- Minimum Certificate / Diploma in Social Sciences (Social Work)
- Relevant work experience in the disability/special needs sector would be an advantage
- Ability to work both independently and within a multi-disciplinary team
- Hardworking, meticulous and responsible.
