Overview

The Job Holder will lead and manage the team under the Clinical Services Planning & Integration (CSPI) Division, and support the RHSO Medical Director to strategise, develop and optimize cross-sectoral end-to-end care plans for conditions with significant disease burden. The incumbent will work with both internal and external stakeholders, including various specialists and primary/community care leaders, alongside cluster Transformation Office, GCMB Office and Group Care Plan Operations to drive and support NUHS’ population health goals and role as Regional Health Manager. The incumbent will also guide and support the CSPI team for grant management of selected Population Health programmes.

Job Responsibilities

(1) Clinical strategy and support for national population / community health initiatives and strategies

  • Support the development of both long and short term strategies and translation of strategies into work plans where relevant, ensuring alignment with overall RHSO and NUHS’ strategies and goals.
  • Ensure effective secretariat support for key national initiatives (e.g. Mental Health; Priority 6A End-of-life care in Nursing Homes).
  • Coordinate and consolidate cluster inputs from specialists and primary care providers for submission to Healthier SG Care Protocols workgroup.
  • Keep abreast of NUHS population health developments and identify opportunities/ gaps/ support where appropriate; Maintain updates and alignment to national and NUHS group care plans, protocols, and guidance.

(2) Support implementation of NUHS Population Health strategies and regional health plan

  • Provide project management oversight, secretariat, and administrative support to drive optimisation of care management for particular conditions/areas, with focus on building and augmenting linkages between community and primary care with tertiary / specialist care.
  • Develop end-to-end care plans and facilitate adoption/implementation of recommendations:Outreach and engagement of relevant stakeholders and KOLs
    Work out theory of change and logic model
    Map existing services, keeping abreast of available NUHS and community services
    Identification of gaps
    Propose recommendations, and facilitate / drive partnerships and collaborations with relevant partners
    Support and monitor development of recommendations
  • Work closely in collaboration with both internal and external stakeholders to actively sync and identify potential areas of collaboration.
  • Sharing of efforts, challenges, next steps and milestones to management.

(3) Work planning and grant management for Population Health programmes

  • Guide and support team for project and grant management of assigned population health programme(s).
  • Work closely with relevant stakeholders including programme team, Legal and Finance to enable smooth work plan development and submission, budgeting, and signing of contractual agreements for both assigned and new programmes tapping on population health funding.
  • Obtain appropriate funding approvals (including for variations), ensure clinical viability and timely submission and processing of claims.
  • Assist MD(RHSO) in clinical sense-making and assessment of proposed initiatives.
  • Support audit / review processes.
  • Support evaluation of programmes and plan for long term sustainability and effectiveness.

(4) Leadership and team management

  • Assist in preparing department's workplans and budget.
  • Manage a team of 5 direct/indirect reports, keeping the team motivated and productive to fulfil and deliver KPIs/milestones.
  • Set priorities and articulate strategies and goals to all direct and indirect reports.
  • Ensure talent development and succession planning, including planning and coordination of training needs.

(5) Any other task as assigned.

Requirements

  • 5 to 8 years of relevant working experience, preferably with 1 - 2 years in a leadership position
  • Experience in healthcare sector and/or clinical operations
  • Experience in project/programme management, community care/population health
  • Possess confidence to resonate with senior management leadership – internal and external stakeholders
  • Ability to lead and influence team members with good coaching/mentoring skills
  • Comfortable working in a fluid environment within a growing organization
  • Ability to multi-task and work under tight timelines
  • Good analytical and organization skills
  • Good command of English, with experience in writing proposals and papers
  • Excellent communication, problem solving, presentation, negotiation and interpersonal skills
