Job Description
To provide holistic care and support to the physical and psychosocial well-being of clients in the DAC.
- Plan and execute daily activities for clients based on AWWA DAC’s programme, according to Person Centred Active Support.
- Provide assistance in activities of daily living (e.g. feeding, toileting, transfers etc.) as required by clients.
- Effectively communicate with Centre Supervisor and team members of AWWA DAC.
- Work within a multi-disciplinary team and family/caregivers to assess, identify, implement and execute services and activities to meet the needs of the respective clients through an individualised care support plan(ICSP).
- Retrieve and maintain accurate programme records of all clients in AWWA DAC in a timely manner.
- Attend meetings as and when required by the programme.
- Responsible for the upkeep and maintenance of all programme resources.
- Assist in ad-hoc projects/committees as and when required by the Centre Supervisor.
Requirements
- At least a Diploma in a relevant discipline, such as Therapy, Sports and Exercise etc.
- Minimum 2 years of experience in the Healthcare sector working with Disability clients.
- Passion to serve the underprivileged.
- Can work independently and in a team
- Excellent interpersonal and communication skills
