Requirements:

· Interpret data, analyze results using statistical techniques and provide ongoing reports

· Acquire data from primary or secondary data sources and maintain data systems

· Work with management to prioritize business and information needs

· Locate and define new process improvement opportunitie

· Provide quality assurance of imported data

· Report the results back to the project managers and stakeholders

· Lead a team of Customer Service Officers to achieve KPI

· Conduct survey interviews via outbound calls and field visits to collect timely and reliable data for national statistical information

· Handle incoming hotline and mailbox enquiries to resolve respondents’ issues

· Compile, record, input survey returns and data accurately into the system

· Conduct data verification of completed survey cases to ensure accuracy and reliability of data

· Set KPI for the officers in accordance to the project timeline to ensure project’s success

· Provide assistance, guidance, and coaching support to officers to meet KPI

· Assess work performance of officers and identify areas that need improvement

· Ensure that the given KPI for the team are being met based on the project timeline

Job Description

• Perform intake assessment and review of day care clients and oversee Multi- Disciplinary Meetings for reviews of clients including Day Rehab clients.
• Supervise daily routines such as meal preparation, serving of meals, preparing clients for activities, conducting of activities, reviews of clients and cleaning duties of care staff.
• Work with centre nurse and care staff to ensure clients specific needs (ADLs) are met and communicated to the team.
• Co-ordinate and plan programmes and services with internal and external stakeholders.
• Direct, guide and participate in the orientation and training of new staff and provide ongoing training and monitoring of care staff in the area of provision of care to day care clients and clients with dementia.
• Take charge of daily operations, and care provisions of clients, supervise and deploy staff to ensure operations of SCC is safe and effective.
• Take charge of incidents in the centre related to staff, clients and volunteers to ensure that the incident is resolved, reported and follow-up.
• Work with Centralised Team to ensure accurate billing and charges for clients and follow up on staff related matters.
• Work with Cluster Lead to ensure financial sustainability of the centre.
• Ensure accurate and timely documentation of communications, assessment and processes
• Carry out other responsibilities and duties as assigned from time to time

Requirement

• Min Diploma / Degree in any field of study from a recognised institution
• Min 4-6 years of relevant operations work experience from a healthcare / eldercare / social services institution
• Prior 2-3 years of working experience in leading a team / holding supervisory or managerial position in the healthcare industry
