B. Daily Duties

For Practical Lessons and Examinations:

1. General cleaning of kitchens including cleaning tabletops, sinks, cookers, sweeping and mopping the floor and emptying dustbins.

2. General cleaning of NFS classrooms including cleaning desks, sweeping and mopping the floor and emptying dustbins.

3. Refill washing detergent, and check and ensure all equipment is in place for each work bench.

4. Teachers to brief the week’s practical and recipe with sample of layout given so that the kitchen helper can start preparations for the classes.

5. Preparation of food trays must be done before the lesson, at least half an hour beforehand.

6. Assist teachers during lessons, help to replenish items, clear teachers’ centre tables, assist students with cookers/ovens/grill/blenders/steamers, mop spills etc.

7. After lessons, kitchen helper to help clean up and store food properly. Any leftover ingredients that are still fresh and safe to use can be kept for use in other classes. Prepare food trays for next lesson if needed.

8. After lessons, check that all gas taps are turned off and cookers are switched off, cookers and sinks are clean, rubbish is disposed of, and clear/clean the centre store table.

9. Laundry for kitchen towels/aprons: Wash them in the washing machine and dry them on the rack. Once they are dry, fold them and place on the laundry shelf.

10. Track and record any breakages.

11. Arrange tables and chairs neatly in classrooms.

12. Check all lights, fans and air-conditioner are switched off in all rooms.

13. Assist teachers in any other duties assigned.

1. Maintenance of NFS rooms

a. Able to maintain orderliness and ensure cleanliness of the NFS food lab (kitchen), and preparation room. Such as:-

i. washing and drying of cleaning cloth, aprons

ii. cleaning of food science equipment,

iii. cleaning of teachers & students’ work area, for instance sinks, stoves, refrigerators,..etc

iv. mopping of kitchen floor,..etc

b. Organize & tidy teacher and student work unit such as table tops, drawers & sinks after each lesson and any other duties related to the maintenance of the NFS room.

c. Manage the equipment and appliances in the NFS food lab(kitchen) (need to be familiar with food science equipment)

d. Assist in checking that the main gas tap, electrical switches (oven) and knives are kept under lock & key.

2. Food Preparation

a. Able to manage the preparation of ingredients for lessons such as the cutting, weighing and packing of ingredients prior to lesson

b. Organise ingredients and equipment in the kitchen which includes the repacking of ingredients into proper storage areas (refrigerator, cupboards).

c. Take stock of perishable food daily and work closely with the teacher-in-charge.

d. Able to operate some of the kitchen and food science equipment such as hand held blender, electric streamer, rice cooker, refractometer etc.

3. Administrative duties

a. Able to manage the process of accepting and verifying food orders.

b. Able to support all NFS teachers in the food lab(kitchen) at all times (excluding lunch time & scheduled breaks)

4. Any other duties assigned by the supervisor

Requirement:

The Personnel shall minimally have the following qualifications, skills and experience:

· Applicants should have completed primary school education

· 3 years of prior experience in food handling, kitchen, or simple administrative duties in similar educational institutions.

· Should be able to work efficiently and independently.

Job Responsibilities*

You will be responsible for the following:

• Attending to medical queries received via a 24/7 integrated telephone hotline that forms a network for triaging patients, and provides an avenue to caregivers and community partners to connect with Alexandra Hospital for information relating to the health and well-being of patients and clients within Queenstown community.
• Ensures that the medical query is escalated to the appropriate medical provider and follows up with proper case closure
• Performs triaging for transitional care referrals and right sites care to other external providers when necessary
• Triage and assess patient's medical-nursing, psycho-social, functional status and daily activity needs; as well as their existing support system availability upon enrolment into programme.
• Provide guidance and assistance to Care Manager Associates in escalation of complex medical calls or referrals triaging when needed.
• Implement appropriate care coordination and transitional case management; and evaluate the outcomes accordingly.
• Synthesize assessment information to prioritize care needs and develop care plan and goals together with patient and/ or family/caregiver; with discussion with patient’s care team as well as community partners involved( if any).
• Work in partnership with patients and families/caregivers on the various ranges of services and available options in the patient’s community. Coordinate and follow up referrals outcome accordingly and in a timely manner.
• Adopt a multi-disciplinary approach with focus on coordination support. Make connections with transitional partners to facilitate support and assistance for individual to address social and health issue
• Conduct follow-up via phone calls and/ or home visits to ensure smooth coping of patients and caregivers.
• Promote and guide positive changes in patient’s lifestyle in the community.
• Monitor patient’s general medical condition during home visit and report to patient’s Principal Physician or primary care provider and/or community partner where necessary.
• Educate and promote advanced care planning, assist patients and their families/caregivers in planning for and improving end of life care, ensuring that choices are reflected in personalized care plans.
• Document assessments, plans, and outcomes promptly and accurately in the relevant system.
• Maintain high level contact with step-down facilities.
• Advocate for patients and their families/caregivers; and form strong relationships with community partners in order to work in the patient’s best interests.
• Participate in activities that contribute towards the improvement of patient care, including professional development sessions to develop relevant areas of knowledge, skills and attitudes.
• Participate in projects and/or community events organized by Alexandra Hospital or partners within Queenstown community.
• Any other duties as assigned by Reporting Officer.

Requirements*

• Degree or equivalent professional qualifications in Nursing, Social Work or Allied Health profession.
• 3 – 5 years of experience in healthcare settings is preferred.
• Knowledge in geriatric and community care will be an advantage.
• Ability to perform 24/7 shift work is required for this role.
