RESPONSIBILITIES

The Communities of Care focus on developing a place-based model of care and building partnerships in local neighbourhoods for the betterment of the seniors. Scope of work could include coordinating social heath services and connecting seniors to relevant service providers to enable them to better manage their health or illnesses. Facilitate care for frail and vulnerable seniors by referring them to home care support and match stay-alone seniors with other seniors and volunteers who are able to support them socio-emotionally.

Key Responsiblities:

• Perform community profiling and identify community needs and assets within the Central/North region of Singapore
• Engage with relevant social and healthcare stakeholders to develop and pilot workflows, protocols and processes to facilitate social-health integration
• Set up linkages and protocols between different community providers to manage clients across multiple social-health services
• Review and track the effectiveness of inter-agency workflow in facilitating care transition and provision
• Develop and implement new initiatives to meet the social and health needs of the community
• Provide information and referral services
• Provide monitoring and coordinate care services for seniors

Bachelor/Graduate Diploma in Social Work or relevant fields (Psychology, Community Work)

Requirements:

• At least 2-3 years of relevant working experience with good knowledge in eldercare or healthcare setting would be advantageous
• High proficiency in written and spoken English with good facilitation and presentation skills; able to speak in mandarin and/or dialects would be essential for engagement with seniors
• Strong interpersonal and communication abilities with individuals and systems at multi-levels
• Team player with an ability to function effectively in a fast-paced environment

Brief Description

Reporting to the Centre Head, the incumbent for Programme Coordinator position provides programme coordination and some administration support for Senior Connect @574.

Job Responsibilities
The job responsibilities for this position include:

1. Assist the Centre Head in providing Active Ageing and Befrienders Services.
2. Plan, coordinate and execute programmes & activities for seniors.
3. Recruit and rally active participation from seniors as assigned.
4. Attend to home visits.
5. Support and update seniors with Information/Referral resources.
6. Participate in other BCARE events and projects when required.
7. Provide administrative support when required. Administrative support includes:
8. a) maintain the database and attendance records of members and volunteers.
9. b) Coordinate schedules and assist in liaising for general repair, maintenance and upkeep of the Centre e.g.
10. air-cons, office and IT equipment, housekeeping and storerooms.
11. c) Assist in obtaining quotations where necessary.

He/she will work closely with his/her Reporting Officer/s to support programme development, professional and administrative functions in the Children, Youth and Family Department.

Job Responsibilities

I. Professional

1. Casework and counselling.

2. Plan and conduct activities for children and youth, both centre-based and school-based.

3. Conduct home visitation to beneficiaries.

4. Manage and liaise with multiple stakeholders in the community to ensure smooth operation of programmes. This includes BCARE’s beneficiaries, volunteers and community partners.

5. Involved in evaluating the validity and viability of programmes.

6. Work with Department Head in planning and implementing new programmes based on the needs of clients.

II. Administrative

7. Ensure timely and appropriate submission of review/outcome reports.

8. Maintain proper records and closure of programmes.

9. Ensure the safe-keeping of important documents.

10. Provide other general administrative support as assigned.

Skills, Competencies and Qualification

1. Must have a Bachelor’s Degree in Social Work, Counselling or Psychology with at least 2 years of experience in

Support in implementing Dementia Singapore Ltd (DSG) palliative care initiatives. Develop a palliative care knowledge management framework that enables DSG to embrace palliative care principles and practices across the care services, including the community care sector at large.

Responsibilities:

• Provide knowledge management support to the Team Lead in the development of a palliative care framework.
• Collaborate with team members and external parties in the development and consolidation of palliative care resources.
• Oversee the development of palliative care materials and resources to advance palliative care practices in the community care sector.
• Ensure information is well presented and easily accessible via different channels and media.
• Maintain and update palliative care information and resources.
• Encourage a knowledge-sharing culture across Dementia Singapore.
• Provide secretariat support to the team lead in organising meetings, i.e., Advisory Committee.

• Degree in Health Sciences or related field.
