Job Responsibilities:
· Partaking in the day-to-day operations in the Eldercare Service at the designated Senior Care Centres or newly awarded Eldercare Service centres.
· Organising social activities and outings for the community at the designated Eldercare Service Centre.
· Working closely with Resident Committees, Community Centres/Clubs and other community partners to source for space to conduct activities and programme.
· Delivering a high level of customer service and engagement with seniors and community partners.
· Conducting targeted outreach and contacting seniors within the Centre’s service cluster at least once annually and befriend or buddy vulnerable/frail seniors once a month or once a week.
· Providing information and referral services to seniors or community in need at the Eldercare Service Centre.
· Maintaining proper records and documentation of networking and collaboration, incidents, phone calls/communications and alert alarm system activations.
· Collecting and compiling KPI information for data reporting and KPI Submission on a weekly/monthly basis.
· Assisting the Manager in data reporting and KPI submission to MOH on a half-yearly or annual basis.
· Assisting the Manager in preparation of MOH Service Audit and competent in the knowledge of the Eldercare Service Requirements.
Job Requirements:
· Bachelor’s degree or diploma
· Min. 1 year of working experience in an eldercare setting or befriending and buddying services [or volunteering services]
· Good communication and interpersonal skills
· Passionate and sincere in planning activities and outreach to active and frail seniors
· Able to work rotating Saturdays
