Roles & Responsibilities

The Social Work Associate / Assistant will provide psycho-social assessment, social work intervention and support to clients and their families to resolve personal, social, emotional, and financial issues.

Duties and Responsibilities

Client Visitation

· Visit clients at their homes to understand their state of emotional and physical health

· Befriend clients and their families

· Accompany clients to medical appointments where required

· Advice clients on other available resources where appropriate

· Guide volunteers and acquaint them with clients during visitation

Client Engagement

· Follow up with non-active clients via phone calls or newsletters

· Encourage clients to participate in 365CPS’ programmes and services

Administrative Support

· Assist clients in applying for financial support from 365CPS

· Prepare reports on visitation updates

· Maintain database to manage details of clients

Job Requirements

· Diploma in Social Work, WSQ Diploma or Certificate in Social Service

· Preferably 1 to 2 years of relevant experience in social service sector

· A team player with good interpersonal and communication skills

· Patience with empathy and strong listening skills

· Proficient in Microsoft Office

Job Description & Requirements

Purpose of Job:

• Work closely with the Allied Health Services Team in nutrition screening and care coordination.
• Promote nutrition literacy and cancer preventive lifestyle among the public.
• Plan and develop community outreach programmes and cancer education workshops.
• Connect clients with cancer to relevant resources and aid beyond 365CPS where appropriate.

Key responsibility areas

Community Nutrition

• Plan, develop and conduct educational programs and workshops, such as public health talks and cooking demonstrations.
• Engage the population to adopt healthy eating behaviour and healthy food choices in the community.

Nutrition Advocacy

• Create educational material about healthy eating habits through marketing collaterals e.g., quarterly newsletter, recipe books, blog posts, etc.

Food Service

• Support Social Service Centre’s kitchen menu revision and evaluation.
• Assist in kitchen food safety and hygiene audit to evaluate kitchen cleanliness.

Referral & External Stakeholder Liaison

• Refer clients to relevant information or resources specific to their needs, including referral to healthcare professionals where appropriate.
• Work closely with the clinical team to ensure best practice and high-quality care.

Requirements

• Diploma in Food Science, Nutrition, Food Service or equivalent
• Intermediate proficiency in Microsoft Office Excel, Word, and PowerPoint

Purpose of Job:

• Care for cancer patients within the Social Service Centre (SSC) according to 365CPS’ mission and values
• Facilitate the smooth running of activities in the SSC
• Provide administrative support to the running of SSC activities

Key responsibility areas:

• Patient Care
  • Attend to clients in the SSC with mindfulness of their needs in terms of diet and rest
  • Encourage clients’ participation in SSC activities
  • Reach out to new clients, including referrals and self-registered cases
• SSC Activity Management
  • Assist in the running and supervision of activities in the SSC
  • Coordinate volunteer support in running DAC activities
  • Engage with activity facilitators and participants to gather feedback
  • Publicise and raise awareness of SSC activities
  • Recommend improvement to SSC activities
• SSC Administrative Support
  • Follow up with absent clients to understand their needs and provide better support
  • Prepare a weekly report on client activity within the SSC

Requirements:

• Minimum 2 years of experience, preferably in events-organising
• GCE O-Level and above
• Basic proficiency in Microsoft Office Excel, Word, and PowerPoint

• Attend to telephone/email enquiries. Refer to the relevant staff for follow up if necessary.
• Mailing/email blasting of marketing materials.
• Support team’s programmes and activities (both offline and online).
• Undertake any other duties assigned by the supervisor.

Requirements:

• GCE O-Level and above
• Minimum 2 years of administrative experience, preferably in a Social Service Organisation
• Fluent in English in both written and spoken
• Basic proficiency in Microsoft Office Excel and Word
