Services include family counselling for conflicts, marital, and parent-child issues; Divorce support for couples and their children; discernment counselling to help uncertain couples decide on their next steps regarding divorce; and online counselling, in partnership with the Ministry of Social and Family Development, for a range of family-related concerns.

Responsibilities:

The department understands the challenges faced in taking care of elderly at home. They ensure there is continuity of care integrating healthcare and social support whenever necessary so that caregivers and their loved ones will be well-supported and enjoy good care at the same time.

Responsibilities:

• Nursing Assessment, developing individual careplan, monitoring and evaluation
• Assess, monitor, review care needs of client and caregiver
• Develop individualised care plan and identify types, duration and frequency of services
• Monitor client’s response to services and evaluate progress towards goals.
• Able to perform nursing procedures such as wound care, tube change, medication management and initiate medical care interventions where necessary

• Ensure proper maintenance of case documentation and timely submission of reports to external parties
• Manage volunteers to support outreach events and conduct activities when necessary
• Establish and support the run of a caregiver support group

Requirements:

• Degree Holder preferably with some social service experience
• Passionate to work in the mental health sector and community services
• Preferably able to converse in Malay or dialect as there is a high proportion of Malay and dialect-speaking clients
• Excellent communication skills and interpersonal skills
• A team player who can also work independently
• Detail-oriented in keeping proper documentation
