JOB SUMMARY

The Social Work Assistant's primary role will be to assist the rehabilitation counsellors in the areas of Medifund Financial Assistance, Case Management support, and/or Chaperone services to medical facilities. 1. Medifund: Assist the counsellors in administrative matters, i.e. compilation of necessary documents and signature procurement.
2. Client support: Provide direct support for up to 15 clients. This may involve conducting pre-screening, intake interviews, assessing clients' needs and concerns, and assisting the rehab counsellors in the development and implementation of service plans.
3. Community re-integration: Assist clients with applications for benefits or resources, connecting clients with community support networks, and ensuring clients receive appropriate care and support during their transition planning.
4. Crisis Intervention: In times of crisis or emergency situations, assist the rehab counsellors to provide immediate support to their clients and/or families. The SWA may assist with assessing the situation, offerring emotional support, and connecting clients with emergency services as needed.
5. Group facilitation: Assist in facilitating support groups or community programs within the centre. This includes helping to plan and organize group activities, ensuring a safe and inclusive environment, and providing guidance and support to participants.
6. Documentation and reporting: Responsible for maintaining detailed and accurate records of client interactions, progress, and services provided. May also assist in preparing reports for supervisors or funding agencies.
