You will be the key contact officer for the Medical Social Service to attend to all telephone and email enquiries. You will provide administrative support to the Medical Social Worker on referrals, accompany social worker on field visits and assist in interpretation if required. You will also gather information and conduct preliminary investigation on patients' needs to facilitate intervention by Medical Social Worker. You will also process application and ensure submission are in compliance.

You are responsible for maintaining the work statistics and compile the data for submission to Ministry of Health. You will also handle external correspondences such as MP letters and CDC letters. You are also expected to participate in projects that improve the work processes in the department and perform any other duties as assigned by your supervisor.

Job Requirements:

• Diploma / Higher Nitec / GCE 'A' or 'O' level.
• Certificate in community and social services or equivalent will be preferred.

Data and Information Management

· Update clientele data and programme information

· Compile data and collate information for management reporting

Outreach Support

· Assist in the coordination of in-house and outreach activities and events

· Support outreach activities at our event booths

· Distribute outreach materials and maintain its inventory

Administration and Reception

· Perform reception and front office duties

· Provide administrative support to designated COMIT and CREST programmes

Requirements

· At least GCE O Level/A Level certificate or Diploma in any discipline

· Preferably some experience in both office administration and front office responsibilities with good telephone etiquette skills

Job Responsibilities

• Responsible for the coordination of care of residents under the care of the General Practitioners (GPs) and Nurse Practitioner (NP).
• Support GPs on primary care initiatives and/or Primary Care Network (PCN) related matters.
• Data entry into database including tracking and coordination to ensure that patients follow the care plan for preventive and chronic diseases as laid out by the GPs and NP.
• Participate in committees or projects to improve care processes related to preventive care or chronic disease management.
• Promote health screening and wellness to the patients through care planning and coordination of care in the community.
• Provide health coaching to PCN patients and work closely with PCN GPs to improve clinical outcome.
• Coordinate social and community referrals eg. Active ageing centre AACs, IRMS services, etc.
• Provide support in quality improvement projects.
• Perform any other administrative duties as assigned by supervisors.

Job Requirements
