The Job

This role is responsible for overseeing Programme Services and Afterlife Care of our elderly beneficairies.

RESPONSIBILITES

• Possess a thorough understanding of Charity’s programs and services.
• When necessary, liaise with relevant bodies to arrange for the member's last rites.
• Create, maintain, and update accurate case files and database of each Afterlife Care members
• Conduct home assessments for potential Afterlife Care members
• Handling of Afterlife Care walk-in or telephone enquiries.
• Following up with cases reported from the hotline or referral from government agencies.
• Explore avenues for the expansion of the Afterlife Care external parties.
• Implement community outreach programs and new projects of eldercare
• Organise and facilitate an elderly befriending program.
• Conduct periodic case reviews and ensure proper documentation
• Refer and coordinate care services, ensuring adequate support to Afterlife Care members.
• Adopt a case management approach when working with beneficiaries, i.e., network with other agencies/specialists to provide multidisciplinary assessments and monitor beneficiaries.
• Prepare monthly reports on the progress of the Afterlife Care programme(s).
• Ad-hoc assistance in the Charity’s events

Requirements

• Minimum Diploma in Social Work or related disciplines
• Prior experience in Social Service setting is preferred
• Able to engage, assess, and provide intervention independently for care plans
• Knowledge of eldercare services, regulations, and practices.
• Ability to work independently.
• Compassion, empathy, and a genuine interest in elder care and charity work
• Proficient in MS Office Applications
• Able to work on some evenings and weekends as needed
• Bilingual in English and Mandarin. Ability to speak dialects will be an advantage

Summary of Role

The Programmes Executive will play a key role in conceptualising and facilitating external talks and workshops, planning and implementing public programmes to enhance mental health literacy, and managing our social media presence. This role also involves content creation, community engagement, and volunteer management and training.

Key Responsibilities

· Conceptualise and Facilitate Talks/Workshops: Develop and conduct engaging talks and workshops for external partners, ensuring content is relevant and impactful.

· Plan and Implement Public Programmes: Design, coordinate, and execute programmes aimed at improving mental health literacy and promoting mental wellbeing within the community.

· Social Media Management:

o Schedule, design, and write content for Clarity Singapore’s social media platforms, including Instagram, Facebook, and Telegram.

o Monitor and analyse social media engagement to enhance our online presence and outreach.

· Community Engagement: Foster relationships with external partners and engage with the community through both in-person and online platforms to promote Clarity Singapore’s initiatives and services.

· Volunteer Management and Training: Recruit, train, and manage volunteers, ensuring they are well-prepared to support Clarity Singapore’s programmes and activities.

· Reports & data collation

· Any ad-hoc tasks and projects in relation to Clarity’s programmes and services

Qualifications and Skills

· Bachelor’s degree in a relevant field (e.g., Psychology, Social Work, Communications, Public Health).

· Preferably 2 years of experience in programme management, community engagement, or a related role. Fresh graduates with relevant volunteering experience may apply

· Strong communication and interpersonal skills, with the ability to engage and inspire various stakeholders.

· Proficiency in social media platforms and digital marketing.

· Creative and strategic thinker with strong organisational and planning abilities.

· Passionate about mental health advocacy and community service.

· Experience in volunteer management is a plus.

• Programme Executive (Active Ageing Centre)

This Programme Executive position is to support seniors to adopt an active lifestyle and pre-empt social isolation. The nature of the job also requires to refer clients to appropriate community care and social assistance when necessary. With Age Well SG initiatives, you shall work with primary care physicians (e.g. family doctor) to empower residents to chart their own journey towards healthy and active lifestyles.

Pre-requisites:

Responsibilities:

• Assist in planning, organising and running of programmes under the guidance of the SPE.
• Work under the supervision of SPE to assist in implementing individual and group activities for residents.
• Assist in preparing the activity room by following prescribed procedures and protocols, including tidying activity areas and assisting in cleaning equipment and area(s).
• Prepare residents for activities by assisting residents into appropriate apparels, and escorting residents to and from the ward and activity room.
• Assist in preparing materials for conducting recreational programmes and events.
• Organise and conduct group activities and excursions.
• Ensure the safety of residents during therapy sessions, activities and outings at all times.
• Assist the Home’s driver in transporting residents from the Home to outing sites and return trips when assigned.
• Guide, supervise and train caregivers and volunteers on their performance of duties (including scheduling) and implementation of activities for the residents.
• To oversee the logistical requirements for the delivery of the programmes.
• Prepare and conduct interactive activities for residents in the Home.

Requirement:

• Diploma or equivalent in any discipline
• At least two years of working experience in a similar position in a Nursing Home
• Able to converse in local dialects
• Passionate in serving the needy and vulnerable elderly
• Have a listening and coaching skills
• Able to work independently

He/She identifies areas of improvements in these programmes, implements quality control and compliance measures and develops information materials for distribution to clients and volunteers. The Programme Executive plans the manpower roster for each programme and guides volunteers in executing programme activities. He/She coordinates with other organisations to execute programmes, manages communications and feedback from stakeholders and conducts outreach activities to other organisations and the community.

Specific Responsibilities:

1. Conduct programmes based on client needs and provide programme updates to supervisor

2. Conduct periodic needs and outcome assessment to evaluate the effectiveness and relevance of our programmes and identify areas of improvements in programmes

3. Implement quality control and compliance procedures, particularly for the physical and psychological safety of our service users, many of whom are young children

4. Maintain daily records on progress of clients

5. Plan the manpower roster for programmes and guide volunteers in executing programme activities by providing briefings, debriefs, and providing support during programmes

6. Develop information materials for distribution to service users and volunteers

7. Coordinate with other organisations for execution of programmes
