Job Overview:

• Role: Care Coordinator
• Location: Redhill
• Salary: Up to $3,000 for candidates with relevant diplomas and internships; up to $3,300 for those with prior experience in Social Work Associate roles.

Key Responsibilities:

• Support Clinical Team: Help clients apply for financial schemes, update renewal applications, and assist in administrative processes.
• Onboarding Process: Manage onboarding of new clients, including scheduling and assisting with subsidies and rehabilitation inquiries.
• Basic Case Management: Help assess client and caregiver needs, with guidance from case managers or social workers.
• Referral Assistance: Assist clients and caregivers in accessing appropriate community services and support.
• Collaboration: Work with multidisciplinary teams and assist with joint home visits and case discussions.

Qualifications:

• Minimum Requirements: Diploma in healthcare management, counseling, psychology, or related fields; internship experience can be considered.
• Experience: Previous experience as a Social Worker Associate or in a similar role can warrant a higher salary offer.

Responsibilities

• Coordinating project schedules, resources, equipment, and information, with the ability to document, monitor, track, and submit measured data.
• Manage the procurement of resources, monitor, and control material usage on-site to prevent wastage or incorrect use.
• Prepare daily reports on work volume, manpower, and materials.
• Organize labor, equipment, and materials for daily site activities.
• Supervise project execution, ensuring adherence to all procedures and method statements.
• Conduct weekly or monthly meetings with subcontractors to update project progress.
• Carry out daily safety briefings and site inspections to ensure compliance with safety regulations.
• Liaise with subcontractors on all project-related matters.
• Ensure project progress remains on schedule.
• Evaluate and resolve discrepancies and/or problems at site.
• Coordinate the submission of incident reports to the loss adjuster.

Other information

• Diploma/Degree in Architecture/Construction Management or equivalent
• Min 2-3 years of experience in building-related projects
• Working schedule: Full 6 days work week for the first 6 months employment (Mondays to Saturdays, 8am-6pm), after that will be 5.5 days work week (Mondays to Fridays, 8am-6pm & Saturdays, 8am-12pm)

Job Description:

- Engage all suitable referrals in Advance Care Planning discussion and facilitate the completion of Advance Care Plans for patients.

- Ensure effective communication of patients’ Advance Care Plans to health care professionals and other stakeholders.

- Follow up and review outcome of Advance Care Planning, including honouring of care and treatment preferences (where possible).

- Follow up on concerns raised during Advance Care Planning discussion, by providing feedback to the healthcare teams and other stakeholders.

- Work in partnership with stakeholders to raise awareness of Advance Care Planning.

- Conduct and participate in Advance Care Planning advocacy and facilitator training.

- Any other duties as assigned by the reporting officer.

Requirements

- Min Bachelor Degree in Nursing, Psychology, Social Work, Counselling or Allied Health relevant to the job.

Job Requirement:

• Conduct site inspections, coordinate with customers, and collaborate with the logistics team to ensure secure and professional equipment relocation.
• Ensure third-party vendors meet customer requirements before service execution.
• Coordinate with third-party vendors for verification, calibration, maintenance, and repair services.
• Provide metrology services using in-house SOPs, ensuring tools are timely calibrated and maintained.
• Respond to ad-hoc service requests within agreed Service Level Agreements (SLA).
• Recommend appropriate inventory types and quantities to minimize equipment downtime and support budget planning.
• Ensure sufficient stock of consumables and chemicals under the Inventory Management (IM) program.
• Maintain accurate documentation for services, including training records, service reports, and vendor qualifications.
• Communicate effectively with customers, vendors, and internal teams to ensure smooth service delivery.

Qualifications and Criteria:

• Diploma or Bachelor’s degree in Science or Engineering, preferably in Electrical or Mechanical Engineering.
