Key Liaison

You are part of the Cancer Survivorship Team, working closely with relevant internal and external stakeholders:

Psychosocial Support includes:

1. Psychosocial Care & Counselling

• Able to develop professional and engaged relationships with clients and other key service stakeholders.
• Provide and facilitate psychosocial assessments to assess cancer patient’s impact of illness and progression.
• Conduct on-going individual and, or family counselling through face-to-face sessions and home visitations to help navigate the bio-psycho-social-spiritual impact of illness.
• Conduct risk assessment for complex cases by considering integration of a range of information independently.
• Work closely with multi-disciplinary teams and, or agencies to deliver care or case plans.
• Lead case conferences as part of a multi-disciplinary team.
• Prepare social reports and ensure documentation is in place.

2. Cancer Survivorship Programme

• To support planning and execution of AYA (Adolescent & Young Adult) Survivorship Programme in partnership with NCCS.
• To support planning and execution of SCS Help the Children and Youth Programme. This includes planning and management of SCS Tuition Programme, annual Award Ceremony, Family Engagement Event and development of non-academic aspects of the programme for our children and youth.
• To work with skilled volunteers and or other stakeholders to support research-related initiatives on survivorship programmes.

3. Other Roles

• Involved in team project that supports community building and development within department and or cross department.
• May be requested to lead, organize and evaluate community development programmes.
• To be involved in review of SCS financial assistance schemes, updating SCS Cancer Care Kit, and or policy changes and future trends impacting areas of work. Be prepared to redesign and adapt programmes to remain relevant in leading the fight against cancer
