• The case management officer reviews patients (inpatient and outpatient) and assess suitability for enrolment under Cardiac Rehabilitation Programme (CRP). Primary Responsibilities

• Input the information into the Rehab Clinic database

• Assume responsibility for data collection and entry into CRP database system.
