The incumbent will be responsible to drive precinct planning efforts to support the development and implementation of strategies and initiatives to enhance the delivery of community care and improve population health in the Western Region. 1. Track progress of Healthy Precinct projects for Western Region, including translation of strategies to implementation plans with internal and external stakeholders.

Summary of Responsibilities:
