As a Case Manager, the job requires you to:
1. Create and Individualised Care Plan for each and every resident; and work towards addressing their psychosocial needs through care and counselling, eventual rehabilitation and reintegration into the community.
2. Assess the residents' needs and risk profile in case planning.
3. Operationalise the care plans for residents and work with other professionals and non-professionals involved in the care of residents.
4. Monitor the residents' progress in the Home and to review their plans six monthly.
5. Conduct induction session with new residents within 3 days upon admission.
6. Conduct home visits for a safety assessment of the home leave environment prior to allowing residents to go on home leave with their families.
7. Conduct monthly counselling session with residents and address issues accordingly.
8. Contact and utilise community resources on residents' behalf, and to serve as a link between the resident and these resource systems.
9. Refer resident to community agencies for post-discharge support at least one month before discharge.
10. Educate and assist staff to better understand the social, emotional, and health issues of the residents
Requirement
1 A degree in Social Work, Psychology, or Counselling
