• Source, negotiate, and coordinate with vendors and suppliers, including catering, venues, audiovisual equipment, and transportation
• Oversee all logistical aspects of events, including site selection, setup, registration, and breakdown
• Collaborate with marketing teams to promote events through various channels, including social media, email, and traditional advertising
• Manage event operations on the day of the event, ensuring everything runs smoothly and addressing any issues that arise promptly
• Act as the primary point of contact for clients, understanding their needs and expectations while providing exceptional customer service
• Conduct post-event evaluations to assess the success of the event, gather feedback, and implement improvements for future events
• Create and maintain detailed timelines and schedules to ensure all event components are executed efficiently and on time

Key Responsibilities:

• Provide comprehensive administrative assistance to senior management, including managing calendars, scheduling meetings, and organizing travel arrangements

Job Summary:

1. Conduct outreach and stakeholders’ engagement.
2. Perform admission and financial counselling.
3. Provide the full spectrum of case/care management service to patients and their caregivers.
4. Support the clinical team in provision of care to the patients and caregivers

Roles and Responsibilities:

1. Engage partners to solicit referrals and stakeholders’ management.
2. Manage patients and caregivers through assessment, development of care plan, execute the intervention plan (Deliver service internally or matching patients to the appropriate services), track and monitor the progress of patients and caregivers.
3. Facilitates patients' placement through admission and financial counselling
4. Familiarize with patient treatment cost and additional costing if there is.
5. Familiarize and understand the patients’ financial resources such as subsidies, insurance, Medisave/MediShield, etc.
6. Conduct visits to dialysis centre, home or hospitals to provide assessment and supportive counselling
7. Advocate for the protection of patient's welfare and rights.
8. Collaborate with internal and external healthcare professionals for a more complete patient management
9. Liaise with government, community, social agencies or individuals to mobilise social resources for the best interest of patients and caregivers
10. Facilitate patient and caregiver support group programmes
11. Review and enhance processes to ensure that they are relevant to the needs of patients and caregivers.
12. Facilitate discharge and Transfer
