Job Summary

The Senior Social Work Care Manager is responsible in the implementation of care plans for clients to meet the Foundation’s mission and operational targets and providing interventions enabling the older person to live in optimal health.

Update and evaluate the progress in timely manner before discharging clients to the longer-term stakeholders in the community as per funder’s requirement.
• To initiates and participates in quality improvement and evidence-based projects and implements quality improvement activities within their own area of work. The staff also contributes to the planning of client/ caregiver education, groupwork and programmes.
• To provides supervision to ACM, interns and contributes to continuing professional development activities for the ACM or peers.

Key Requirements

• Post Graduate Diplomq in Social Work
