Key Responsibilities:

STATEMENT OF PURPOSE

This role support and assist the nursing team and the medical social work team in the administrative tasks, information gathering, form filling, fieldwork and other ad hoc duties.

KEY RESPONSIBILITIES

1. Pre-admission/Admission Support

1.1. Orientate new residents and their NOKs to the nursing home

1.2. Ensure residents settle well in the nursing home.

2. Casework (non-complex case)

2.1. Engagement and Assessment

• Establish rapport and build professional relationship with resident and significant others

• Identify and clarify the key issues to the problem situation and gather relevant information from resident and NOKs

• Conduct bio-psycho-social-spiritual assessment to assess and identify the severity and key risks and protective factors for intervention

• Engage residents, families, multi-disciplinary team and relevant stakeholders to gather holistic and relevant information

2.2 Goal Setting and Intervention Plans

• Analyse and evaluate different courses of action, when appropriate with multi-disciplinary team and relevant stakeholders

• Formulate appropriate strategies for action

• Conduct care planning and goal setting with residents based on standard guidelines and professional assessments

• Set clear contracts and goals with residents - Engage residents to sustain effort in problem-solving

• Apply appropriate assistance schemes

• Collaborate and negotiate with stakeholders to mobilise community resources

2.3 Case Review and Documentation

• Follow up and review care plans and provide recommendations to modify care plans where necessary

• Evaluate the process as well as the outcome objectively and recommend future improvement

• Provide documentation and timely updates of assessments and intervention to be used by multi-disciplinary profession

• Work with MSWs on providing social reports with proper consent to relevant

stakeholders to ensure continuity of care

3. Keep residents meaningfully engaged

3.1. Understand interests of individual residents

3.2. Organise activities that are of interest to residents

3.3. Engage and motivate residents to participate in the activities

3.4. Work with volunteers and Volunteer Manager in activities planning and implementation

3.5. Review and evaluate the effectiveness of activities in improving the well-being of residents or handle the situation.

4. Support volunteer program

4.1. Provide feedback on possible new volunteering opportunities based on the needs observed on the ground.

4.2. Work closely with the Volunteer Manager on effective mobilising of resources to support volunteer program.

4.3. Activities Programme

• Provide supervision during implementation of activities

• Provide feedback to volunteers on areas of improvement via Corporate Communication in charge

5. Support Advance Care Planning (ACP) program

• Initiate ACP awareness with residents and their NOKs

• Facilitate General ACP or PPC with residents and their NOKs for those who are ready

• Review ACP plans regularly

• Assist residents in fulfilling their care preferences and last wishes.

ADDITIONAL DUTIES

1. The staff will undertake projects relevant to the post, as requested by HOD or ED.

1. BROAD FUNCTION

Manage referrals from AIC and direct referrals from community partners and general public. Provide end-to- end case management support and coordination to ensure a smooth admission process and service delivery for home care clients that require Home Personal Care, Home Nursing, Home Therapy, Home Medical and Medical Escort services, etc.

Responsible for ensuring the smooth functioning of the department by providing administrative support to the team.

2. RESPONSIBILITIES

Client Management

• Manage home care client data base to ensure completeness and accuracy.
• Process home care referrals through AIC referral system as point of liaison.
• Perform basic financial assessment for clients or next-of-kin on home care service(s) required and refer to social worker if financial assistance is required.
• Perform basic assessment of care needs for home care clients and conduct home visits where required.
• Coordinate internal and external referrals of services required (e.g. referring home personal care client for home nursing or medical social worker).
• Coordinate with volunteers to engage home care clients meaningfully.
• Coordinate and participate in multi-disciplinary discussions to address the social and clinical needs of the clients.
• Respond to complaints and feedbacks from clients and next-of-kin.

Administrative duties

• Manage and schedule deployment of care staff/clinicians for clients based on their preferred date, time and frequency of service, etc.
• Check and verify invoices based on the services rendered every month.
• Collect relevant and accurate data for reporting to management and regulatory authorities.
• Participate in project work (e.g. quality improvement within department/organisation) to improve the quality of care and services to patients.
• Identify service gaps in response to clients’ needs and make recommendations to improve service provision of home care.
• Process senior mobility fund (SMF) applications for clients upon forms completion by nurse/therapist.
• Verify and submit quarterly SMF claim submission.
• Generate monthly invoices for client services and liaise with Finance/Business office to ensure transactions are accurate.
• Verify and process monthly invoices from vendors/service partners to ensure smooth payment of services.
• Verify and submit invoice details and attendance in ILTC system for subvention claims every month.
• Verify and submit quarterly ILTC subvention report.
• Submit medisave claims for clients every month.
• Any other duties as assigned by reporting officer.

Requirements

• At least a Diploma in Nursing / Allied Health / Psychology / Counselling /Social Work / Sociology/ Community & Social Service (Eldercare) is preferred.
• At least 1 year of working experience in ILTC sector.
• Able to use local languages and dialects.
• Proficient in Microsoft Word, Excel, PowerPoint and Outlook.
• Must be client-oriented, self-directed, independent yet able to work as part of a multi-disciplinary team and have good interpersonal and communication skills.
• Excellent time management skills, well organised and outcome focused.
• Able to work in a fast paced and dynamic environment.
