RESPONSIBILITIES

• Receive and assess referrals to ensure appropriate placement in Centre.
• Provide end to end casework management and psychosocial support services to residents, clients, and their families. Utilize appropriate frameworks for holistic assessment.
• Identify and develop individualized care plans together with clinical care team to meet the psychosocial and emotional needs of residents and clients.
• Address issues related to grief, adjustment, and coping with chronic illness or disability.
• Implement and facilitate appropriate casework interventions, including ongoing monitoring and evaluation of outcomes.
• Establish network with external partners and stakeholders for resourcing and ensure continuity of care of residents/clients across settings.
• Work closely with multidisciplinary team to ensure that holistic needs of the resident/client are actively considered in their care planning.
• Ensure "hands-on" approach to working with residents/clients through individual work, group work, counselling, or other appropriate modalities.

• Bachelor Degree in Counselling/Social Work/Psychology or related discipline.

• Preferably with at least 2 years' work experience in social services field (MOH or MSF) performing case management tasks.
• Experience working in a healthcare setting and knowledge of healthcare financing will be a plus.
• Able to work independently as well as be a team player.
