Job Description

You are expected to perform social and community-based work in the areas of intervention and programme support, community development and implementation, and identify gaps for programme evaluation and integration of existing programmes and services.

RESPONSIBILITIES

1. Plan, implement and monitor the care process, referral and follow-up workflow.
