DUTIES AND RESPONSIBILITIES

1. Case work

1.1 Role in use of Community Screening (as part of company team)

1.2 Role in Case Assessment

1.3 Role in Intervention and Care Planning

1.4 Care in providing general and clinical SW support to clients

1.5 Role in Information and Referral

1.6 Create and maintain casework records for elderly household in assigned areas.

2. Group Work

2.1 Role in determining psycho-education / support / therapeutic needs seniors groups.

2.2 Role in formulating SW group work in our company

2.3 Role in overseeing general group work / activity of the company

2.4 Role in the conduct of SW group work and general group activities

2.5 Role in evaluating general group work in the company

3. Community Work

3.1 Assist in the determining of the community needs of the senior population within

company boundary.

3.2 Assist in the development of the company's Community outreach and Community

development plans – for the short, medium and long term well-being of the

seniors in the Company boundary.

3.3 Identify and establish network of resources within the community.

3.4 Assist in the carrying out of the company Community Work plan.

4. Programme Planning and Implementation

4.1 Assist in the development and implementation of outreach plan to seniors.

4.2 Assist in the development and implementation of the company’s annual calendar plan of

social and health activities for seniors.

4.3 Ensure activities and programmes of the company promotes local community aspirations

5. Others

5.1 Alert Alarm System

• Assist in the monitoring and management of the Alert Alarm System (AAS) during

the centre’s operating hours, rendering assistance when necessary.

5.2 Volunteer Development

•To assist in the development and coordination of volunteer activities.

5.3 Training and Development of Personnel

•Lead, motivate and train staff volunteers where possible.

5.4 Self Care Plan

•To develop and implement an approved Self Care, in order to provide optimum

service standards for the client, colleagues and agency .


Job Specifications:

•Qualifications: Degree /Post Graduate Diploma in Social Work

•RSW is preferable or in process
