RESPONSIBILITIES
You will be responsible for the planning and execution of group activities for the Fun with Friends Programme and coordinating with internal and external stakeholders including AAC staff, volunteers, and activities facilitators or occupational therapists, in ensuring smooth running and operations for the programme.

Principal Accountabilities
- Implement various multi-modality group activities for persons with dementia and ensure the smooth running of the programme. Maintain a high standard of integrity and professionalism in the delivery of quality client and customer-centric care.
Fun With Friends Programme
- Implement and facilitate various multi-modality group activities for persons with dementia and cognitive impairment as outlined in the manual; including exercise sessions, meal preparation groups, cognitive games and music and art-related activities.
- Assist in activity planning and preparation to ensure the smooth execution of the group activities
- Manage clients with different mental, and physical conditions and behaviour of concerns relating to dementia
- Administer assessment to clients and caregivers, perform data collection and entry
- Perform timely documentation for the client post-session
- Manage client's database and updating of the client's attendance
- Provide timely updates to family/NOK of client's engagement and progress in the programme
- Provide customer support to customers, partners and other external stakeholders, which includes but not limited to enquiries, enrolment, service coordination and feedback
- Cover other executives or programme facilitators in their absence when needed
- Support and or conduct training to staff or volunteers relating to assist with running the programme
- Precept new programme team staff, and provide necessary guidance and documentation if assigned.
Senior Care Centre Programme - Adhoc Project Support
- Assist in senior day care programmes;including IGP/volunteer program as assigned which include but are not limited to administrative and logistical support and keeping track of expenditure
- Administer assessment and survey to client, caregiver or staff in relating to programme
- Assist the manager in tracking progress and evaluating the programmes
General
- Manage inventory and assist in the procurement of activities materials, with approval sought from RO/programme in charge
- Provide basic housekeeping of the programme premise
- Monitor and tracking of budgeting and expense records. Submit quotations and invoices for the programme to finance
- Ensure timely and effective communication between internal and external stakeholders (e.g. therapist, exercise trainers and activities facilitators, programme team, volunteers and active aging centre staff) in designing and running collaborative programmes
- Assist with internal and external audits to ensure the programme meets the service model requirement
- Prepare timely reports to the management and funders of the programme
- Assist in outreach and networking events to promote the program, generate new interest and recruit new client
- Minimally a Diploma in social service or related discipline
- Possess working experience in eldercare, health care, therapy-related area
- Well-versed in MS Office
- Comfortable working and communicating with person with dementia
- Comfortable in running group sessions and programme
- Strong interpersonal and communication skills
- Ability to speak English, Mandarin and dialects will be an added advantage
- Willing to travel to different centres island-wide as needed
