We are seeking a highly motivated and dedicated Primary Care Coordinator to join our team in Singapore. As a Primary Care Coordinator, you will play a vital role in supporting the coordination and delivery of primary care services within our Primary Care Network (PCN). You will work closely with healthcare providers, patients, and other stakeholders to ensure seamless care coordination and enhance patient outcomes.
Responsibilities:
As a PCN Primary Care Coordinator, you will be responsible for:
- Coordinating services and appointments for patients within the PCN, such as diabetic foot and eye screening, nurse counselling and chronic disease management.
- Providing administrative support to the PCN team, such as (1) managing referrals, reports and claims, (2) communicating information on various subsidies, schemes, community resources, (3) analyzing data and conducting quality improvement projects, and (4) ensuring compliance of clinical requirements outlined by MOH.
- Educating and empowering patients on medication management and lifestyle changes, as well as advocating health promotion and preventive health.
- Building rapport and trust with patients and their caregivers, and facilitating communication among the PCN team members.
- Working with General Practitioners (GPs), Family Physicians (FPs) and their clinics on ancillary services and referrals.
- Supporting the implementation of the Healthier Singapore (HSG) initiative, which aims to help Singaporeans take steps towards better health through a choice of their preferred family clinic, a customised health plan and access to community programmes.
Qualifications:
To be a PCN Primary Care Coordinator, you should have:
- A diploma or degree in any discipline, preferably in health sciences or related fields. Fresh graduates from related fields are welcome to apply.
- At least 2 years of relevant working experience in healthcare or social service settings (for non-fresh graduates).
- Good interpersonal and communication skills, both verbal and written.
- Proficiency in Microsoft Office applications and basic IT skills.
