• Responsible for the triage of specialist outpatient appointment requests received from various referral streams to divisions under Department of Medicine.
• Calling patients to confirm their attendance and assist them to reschedule or cancel the appointment.
• Communicate and coordinate with referring healthcare stakeholders regarding the outcome of the outpatient referrals request made.
• Collect and evaluate appointment wait time data for review by department and assist in preparation of management reports.
• Participate in projects to improve processes related to appointment wait time management.
• Any ad-hoc duties as per assigned.

Requirement:

• Minimum Bachelor's degree in Science or Bachelor's degree in Nursing

Job Scope:

• Responsible for the triage for specialist outpatient clinic appointment requests received from different referral steams
• Consolidate appointments for patients where appropriate
• Assist to confirm patients’ attendance especially for first visit appointments, and assist in cancelations, rescheduling or ad hoc patient enquiry
• Contribute to the implementation of department plans to right site outpatient referrals via risk stratification on the basis of available historical clinical, biochemical and radiological information
• Work closely with referring healthcare stakeholders regarding the outcome of the outpatient referral request made
• Assis to plan outpatient visit as per available clinic slot based on estimated urgency to be seen
• Collate and evaluate appointment waiting time data for department review
• Responsible for proper documentation of appointment allocation and triaging data, with adherence to PDPA policies
• Ad hoc duties as assigned

Job Requirements:

• Minimum a Degree in Life Science
• Registered Nurses with a Degree in Nursing are welcomed to apply
• Relevant working experience in healthcare industry would be an added advantage
• Competent in MS Office
• Good communication and interpersonal skills

Job Scope:

• Carry out discharge plan assessment for inflight cases to facilitate and expedite patient discharges to community hospitals
• Provide timely information to patients and family members and assess their needs when choosing a suitable community hospital
• Document the assessment, care plans, and outcomes timely and accurately in the appropriate IT systems
• Monitor the referral trends and identify service gaps and any transfer delays
• Provide supervisory, guidance and foster collaboration among team members
• Develop career progression pathway for team members and competency test of the team
• Perform any other duties as assigned

Job Requirements:

• Bachelor Degree in Health Science / Nursing or related fields from a recognized institution
• Preferably with 5 years of experience in a supervisory role
