1. Job Role

• The Head of Case Management oversees a team of case workers responsible for resident care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and manages the department as per the regulatory practices and policies.

2. Job Description

• Responsible for direction, coordination, management, review, implementation and evaluation of resident case management for the organisation in alignment with the organisation’s strategies and policies and procedures and directions of higher management

• Bachelor’s Degree in Counselling or Psychology
