Job Description:

• Patient Care: Work with the healthcare team to provide care based on patient needs and conditions.
• Admissions and Discharges: Coordinate patient admissions, transfers, and timely discharge planning.
• Patient Progress: Monitor and evaluate patients' recovery and readiness for discharge.
• Collaborative Care: Partner with the team to deliver evidence-based treatments for optimal outcomes.
• Condition Monitoring: Watch for changes in patient conditions, recognize early signs of issues, and escalate appropriately.
• Medical Rounds and Procedures: Join team rounds, carry out doctors' orders, and assist with procedures.
• Communication: Update the team on patient progress and response to treatments.
• Clinical Skills: Use critical thinking and expertise to deliver quality patient care.
• Problem Solving: Support the nursing team with clinical or operational challenges and escalate when necessary.
• Patient Education: Educate patients and families about their conditions and self-care.
• Resource Management: Help manage resources efficiently and stay within budget.
• Infection Control: Review and improve infection control practices.
• Training and Development: Conduct training for nurses and students to enhance their skills.
• Change Implementation: Support new initiatives and lead changes within the department.
• Feedback and Supervision: Assess and provide feedback on the performance of new nurses and students.
• Clinical Resource: Offer guidance on medical equipment, systems, and workflows.
• Emergency Support: Help lead responses during clinical emergencies.

Job Requirement:

• Min Degree in Nursing
• Min 3 yrs of relevant perm inpatient & senior nursing experience, preferably in gerontology/palliative care
• Preferred in Advanced / Specialist Diploma in Geriatric / Community Nursing
